Pledge form - fillable Instructions:

1. Click here to open the pledge form*
2. Enter your pledge details on the form
3. Review your pledge information and click the "Submit by Email" button at the bottom of the form.
4. If an option to send by "Desktop" or "Internet" email program comes up, go ahead and select "Desktop" and press "OK".
5. This will automatically open your email program and attach your form. You can enter the email address of your campaign coordinator and simply press "Send" in your email program.
6. Press the "Print" or "Save" icons to save a copy for your records.

*  This form requires Adobe Acobat Reader v 7.0 or later.  Mac Preview may not allow data entered into the form to be transmitted via email.
